Transitioning Provider Reassessors to Fusion

As you know, DSDS is preparing to go live with Fusion, the new HCBS electronic case management system that will replace Web Tool. As part of the transition process, new user accounts will need to be made for provider reassessors.

In Fusion, Provider Reassessors will have a different user role from provider users that are not reassessors. In order for DSDS to accurately provision user accounts for those reassessors we need updated information. If your agency is actively participating under the Type 27 Provider Reassessor contract, you will need to respond to the survey sent to the business email address listed with MMAC. The survey must be completed by April 18, 2025. Only one person from your agency should complete the form on behalf of all active provider reassessors working for your agency. Type 27 providers who fail to respond will not receive assignments.
